- Be eligible under Title II or Title XVI of the Federal Social Security Act for benefits for the disabled;
- Has been a New Hampshire resident for at least five years as of April 1 and own and occupy the real estate individually or jointly, or if the real estate is owned by a spouse, they must have been married for at least five years.; and
- is living in the home.

- Most recent bank statements ALL PAGES from ALL BANKS which include, checking, savings, stocks, bonds, mutual funds, money markets, IRA’s, Statements for CD’s, 401K Statements, etc.
- Life Insurance Policies
- Dividend Statement(s) and/or Interest Income Statements
- Federal Income Tax Return (or W-2 Forms)
- Trust Document (if applicable)
- PA-33 (Statement of Qualification) if property is under a Trust.
